This project delivers four key diagnostic services—24-hour Ambulatory Blood Pressure Monitoring (ABPM), 12-lead Electrocardiograms (ECG), Phlebotomy, and Spirometry—within primary care settings across Wandsworth.
These services aim to enhance early diagnosis, monitoring, and management of chronic and acute conditions, while improving access and convenience for patients.
We are a commissioner of these services, and our role involves contract monitoring the service delivery by Wandsworth GP Practices for their patients.
All four services are being delivered under contracts running from 1 April 2025 to 31 March 2026. The service was first commissioned by the Federation in 2016.
Key partners involved in the service, include:
- South West London ICB
- Wandsworth GP Federation
- Local acute hospitals and pathology departments
- Cardiology and respiratory units
- Community Respiratory Nursing Specialist Service
- Third sector organisations
- Wandsworth Borough Council
- Healthwatch and service users
These partnerships support integrated care pathways, shared clinical protocols, and coordinated service delivery across the borough.
The service benefits patients and service users in the following ways:
- providing timely access to essential diagnostic tests close to home
- Reduced waiting times (within 2–4 weeks depending on the service)
- Improved diagnosis and monitoring of conditions such as hypertension, COPD, asthma, and cardiovascular disease
- Convenient and accessible care in familiar primary care settings
- Enhanced patient experience through feedback-informed service design
For primary care and the wider healthcare system, the benefits of the service are:
- Cost-effective service delivery within primary care
- A reduced burden on secondary care and hospital diagnostics
- Improved clinical decision-making through timely access to diagnostic data
- Streamlined care pathways and improved continuity of care
