We lead a range of digital and change management programmes designed to modernise primary care and improve patient access. Our work aligns closely with NHS England’s plan for recovering access to general practice — with a strong focus on empowering patients and enhancing service delivery.
Empowering patients
We’re helping patients take greater control of their health by making digital tools more accessible and inclusive:
- Boosting NHS app adoption
We support practices in promoting the NHS App, which gives patients easy access to services via mobile, tablet or desktop. In Wandsworth, PCN averages of patients aged 13+ has reached 66%, surpassing the national average of 59% and a regional average of 64% (June 2025). - Enabling access to online records
We work closely with GP practices to help them manage and expand patient access to their online medical records. - Championing digital inclusion
In partnership with Wandsworth Borough Council, SWL ICB, Healthwatch, Age UK, Power 2 Connect, and Putney Library, we ensure residents can use digital health services safely and confidently. - Supporting the Pharmacy First programme
This initiative encourages patients to visit community pharmacies for minor illnesses and specific conditions — making care more convenient and accessible, whilst ensuring those who need to speak with a GP, are able to.
Modernising Access to general practice
We’re helping practices evolve how they deliver care, making services faster, smarter, and more responsive:
- Improving care navigation and response times
By streamlining triage and assessment, we’re helping practices boost capacity and resilience — while making the most of their multi-disciplinary teams. - Providing guidance, training and hosting showcasing events
We do this to support practices to optimise their current processes and onboard more effectively onto new systems, for safe and sustainable innovation. - Enhancing digital telephony
We support the rollout and optimisation of digital phone systems to improve the patient experience when contacting their GP. - Simplifying online registration
We assist practices in adopting NHS England’s Online GP Registration service, making it easier for patients to register without needing to visit in person.
