Empowering patients
We’re helping patients take greater control of their health by making digital tools more accessible and inclusive:
- Boosting NHS App Adoption
We support practices in promoting the NHS App, which gives patients easy access to services via mobile, tablet or desktop. In Wandsworth, uptake among patients aged 13+ has reached 61%, surpassing the national average of 57% (as of May 2024). - Enabling Access to Online Records
We work closely with GP practices to help them manage and expand patient access to their online medical records. - Championing Digital Inclusion
In partnership with Wandsworth Borough Council, Healthwatch, Age UK, Power 2 Connect, and Putney Library, we ensure residents can use digital health services safely and confidently. - Supporting the Pharmacy First Programme
This initiative encourages patients to visit community pharmacies for minor illnesses and specific conditions — making care more convenient and accessible.
Modernising Access to general practice

- Improving Care Navigation and Response Times
By streamlining triage and assessment, we’re helping practices boost capacity and resilience — while making the most of their multi-disciplinary teams. - Enhancing Digital Telephony
We support the rollout and optimisation of digital phone systems to improve the patient experience when contacting their GP. - Simplifying Online Registration
We assist practices in adopting NHS England’s Online GP Registration service, making it easier for patients to register without needing to visit in person.
