We provide support and oversight of end-of-life care services in the community and primary care across Wandsworth.
This involves working closely with practices, Central London Community Healthcare community nursing service, specialist providers like Royal Trinity Hospice and Marie Curie and acute palliative care services to ensure people receive the care they need as they reach the end of their lives.
We do this in a number of ways, including:
- Supporting practices provide end of life care, in particular ensuring awareness of the pathways available and enabling them to plan care. This involves ensuring that the wishes and care needs of patients and their families are understood and care plans are identified and recorded appropriately
- Reviewing feedback from patients, their families and other stakeholders to review their experiences and facilitating changes to how service are delivered where necessary
- Monitoring the outcome and quality of services
- Providing opportunities for services to share knowledge and learning to improve the provision of care and communication
- Facilitating learning opportunities and resources across the borough to support staff to deliver high quality care
- Working with partners across South West London, regionally, and nationally to ensure that services incorporate evidence-based and best practice guidance and meet the priorities for palliative and end of life care services.
