The PACT (Planning all Care Together) programme aims to improve outcomes for vulnerable patients through proactive care planning, collaboration, and targeted interventions across a range of clinical areas.
We are the commissioner of this project from Wandsworth GP Practices for their registered patient population. The programme started in 2015 and the current contract period runs from 1 April 2025 to 31 March 2026.

We collaborate with our partners through shared care planning, taking part in multidisciplinary team (MDT) meetings, sharing data, and coordinating patient pathways. Practices engage with their stakeholders to validate patient cohorts and deliver integrated care.
The service benefits patients and service users in the following ways:

- Enhanced support for carers and patients with mental health, diabetes, and end-of-life needs
