As we mark Heart Month, we’re proud to shine a light on our outstanding work as a London demonstrator site using the CVDAction tool.
Cardiovascular disease (CVD) is one of the leading causes of avoidable mortality in the most deprived communities in Wandsworth. CVDAction is a smart data tool developed by UCL Partners which supports GP practices and Primary Care Networks with early detection and prevention of Cardiovascular risk factors.
Across Wandsworth, West Wandsworth, PRIME and Battersea Primary Care Networks delivered clinical activity guided by data generated from the CVDAction dashboard.
The impact of CVDAction as a smart data tool is currently being evaluated by NHS England with early results showing that, in Wandsworth, five times more patients had their blood pressure control optimised than any other site.
