CVD is one of the leading causes of heart attacks, making prevention and early detection essential to improving heart health.
Across Wandsworth, we’re proud to support the following strands of work and the difference they’re making for patients, service users and the wider healthcare system. Together, they mark significant progress toward tackling and reducing cardiovascular risk and improving heart health during Heart Month and into the future:
CLEAR Living Well 3
The CLEAR (Clinically Led Workforce and Activity Redesign) Living Well Programme is commissioned and funded by NHS England and delivered in partnership with 33N.
33N’s team of NHS clinicians, education specialists and data scientists co deliver the National CLEAR Programme supporting PCNs nationally to design new models of care for the prevention of CVD and to identify patients at risk earlier using population data.
As part of this work, Wandsworth PCN successfully completed the programme in November, where they focused on reviewing essential hypertension, a major contributor to CVD. Through two co‑design workshops, they redesigned and standardised a hypertension detection and management pathway for adults aged 18–79, improving consistency, early diagnosis, and the quality of follow‑up care.
A dedicated CVD Prevention Team is now in place, and the redesigned pathway will be piloted across two practices, with wider rollout planned following evaluation.

As part of Heart Month activities, Wandsworth Borough Council’s Public Health team has confirmed dates with Wandsworth PCN to utilise the community health bus for Health Checks and blood pressure readings in the community between the 25th- 27th of March 2026. More details to follow shortly.
Supporting Improvement Nationally
We’re also proud to support improvement nationally as part of the CLEAR programme.
Supporting Coast and Country PCN in Devon ICB through the programme, drawing on CLEAR’s nationally recognised, clinically‑led methodology to contribute to progress and share learning as they redesign their hypertension pathway.
This collaboration supports the spread of best practice across NHS systems.
CVDACTION Pilot – West Wandsworth PCN
West Wandsworth PCN have participated in a CVDACTION programme as a demonstrator site for the CVDACTION Data Dashboard, focusing on hypertension detection and management. This work was supported by the Wandsworth GP Federation during the pilot.
The CVDACTION demonstrator programme, developed by UCLPartners and delivered through Into-Action.Health has shown how smart data and structured implementation support help primary care teams identify high risk patients more effectively and optimise treatment.
As part of the demonstrator work, West Wandsworth PCN adopted creative engagement methods, including tailored patient communication scripts, resulting in notable improvements in hypertension control compared with other demonstrator sites. This reflects wider evaluation findings highlighting the importance of psychological and social factors in supporting patient engagement.
