To do this we follow the national Patient Safety Incident Response Framework (PSIRF), which guides the NHS on how to investigate and understand patient safety incidents in a fair, consistent and compassionate way.
This framework helps us focus on learning rather than blame, so we can identify what went wrong, why it happened, and what improvements are needed to reduce the chance of it happening again, ensuring better care for all our patients.’
